Our Programs

Northern Territory PHN (NT PHN) integrates and coordinates a diverse range of primary health care and population health services that are provided by other organisations in the public, private and not-for-profit sectors. We achieve this by working collaboratively with health service providers to strengthen the capacity of the primary health care system itself and its interface with hospital care. We also engage directly with communities and consumers to better understand their health care needs as a basis for developing and enhancing health services.

Our main functions include:

  • engaging with the community and service providers in health needs assessments, planning and service coordination
  • health workforce planning and support
  • managing funded health and capacity building programs. 

Health Programs

NT PHN programs branch administers a number of funded programs that commission primary health care services through a range of providers across the Territory. Services provided span mental health, allied health, preventative health, refugee health and Aboriginal and Torres Strait Islander health.

Practice Support Programs

NT PHN's Practice Support team delivers programs that are relevant to the day-to-day operation of primary health care organisations in the NT.  As well as developing and linking these organisations and their staff to the appropriate information, tools and resources to support the highest possible quality of service delivery, the Practice Support team also plays a vital role in linking these organisations through networking opportunities and collaboration of program delivery.
