If you have a chronic or complex health condition, like diabetes or heart disease, you may be able to use your local general practice or Aboriginal Community controlled health service as your health care home. This means your clinic will help coordinate your care with all the various health professionals you need to see.
In the NT, 25 clinics are part of health care homes. If your clinic is a health care home, ask them if you’re elgible to join the program.
