Northern Territory PHN (NT PHN) invites suitably qualified candidates and consortia to apply for funding under the After Hours Primary Health Care Innovation Grants for initiatives that aim to increase the accessibility and effectiveness of after-hours primary health care in the Northern Territory.
After-hours primary health care services are services that meet the need for unexpected after-hours medical conditions that cannot wait until general practice hours.
The After Hours Primary Health Care Innovation Grants offer funding opportunities in the following categories:
- Individual Organisations to a maximum of $400,000 (GST exclusive)
- Consortium Applications to a maximum of $900,000 (GST exclusive).
To be supported, projects must either:
- develop and deliver a model of service in after-hours primary health care, and/or
- be focused on system reform, and/or
- develop and deliver health literacy/education to support the use of after-hours primary health care.
The After Hours Primary Health Care Innovation Grants is available for local projects or activities that address key priority areas that relate to the delivery of after-hours primary health care across the NT.
Projects must commence by 1 July 2016 and be completed by 30 June 2017.
