Severe and Complex Mental Illness

NT PHN is funded by the Australian Government to commission primary mental health care services, including clinical care coordination for people with severe and complex mental illness. NT PHN is establishing the Mental Health Nurse Program in the Northern Territory to meet this need.

The Mental Health Nurse Program provides clients with severe and complex mental illness access to mental health nursing care. The Mental Health Nurse Program will be located within eligible organisations with the ultimate aim of clinical care and coordination delivered by Credentialed Mental Health Nurses (CMHN) to support clients to effectively manage their symptoms and avoid unnecessary hospitalisation. The CMHN is able to deliver this clinical support service within a clinical setting, the community and in their home environment, with no set time or session limitations.

Service Providers are contracted to deliver this service through employment of a CMHN.
